Before Your Facelift Surgery

Dr. Cho will perform a thorough assessment of your health and medical history before your facelift. He will answer your questions and give you pre- and post-operative care instructions. You will also receive a prescription for pain medication and antibiotics so that you may have your medications handy prior to surgery.

Before your facelift, you will be instructed to avoid tobacco products and certain blood-thinning medications and supplements. You will also be instructed to avoid alcohol in the weeks and days prior to your facelift. You will be asked to report any new health concerns or illnesses to Dr. Cho immediately before your procedure so he can be sure you are healthy enough for surgery.

Our Facelift Approach

Facelift surgery is typically performed while patients are under general anesthesia, so you will be asleep during your operation and will not feel anything. The procedure typically takes between two and five hours to complete. In addition to lifting and repositioning your underlying facial muscles and eliminating excess skin, Dr. Cho will also perform additional techniques to create the results that you envisioned during your preoperative consultation. The Five Rs of Facial Rejuvenation

Our signature approach to full facial rejuvenation, coined the “The Five Rs to Facial Rejuvenation,” is designed to globally address the signs of aging and achieve long-lasting results. Each R works together to restore different layers of the face using a combination of surgical and non-surgical procedures. This advanced method includes relaxing muscles, replenishing lost volume, restoring the skin, repositioning the tissues, and reducing facial volume.

R1 – Relax Muscles

We help to relax the muscles and smooth out the appearance of wrinkles by addressing the forehead, crow’s feet, frown lines, masseter lines, chin, and lip lines with muscle relaxer treatments like Botox, Dysport, Xeomin, and Daxxi. Muscle relaxers also help to reduce migraines, excessive sweating and gummy smiles.

R2 – Replenish Lost Volume

Whether you have volume loss from weight loss or due to the aging process, we can help replenish the volume and contour the face with fillers like Juvéderm, Restylane, RHA, Sculptra, or Radiesse. When combining relaxation of the muscles (R1) with replenishing lost volume (R2) it can make a tremendous difference. We can enhance your results when using R2 in combination with volume reduction techniques (R5), repositioning of the tissue (R4), and restoring the skin (R3).

R3 – Restore the Skin

At Integrated Aesthetics, we offer a robust portfolio of skin resurfacing and tightening devices to help improve the texture, health, and appearance of your skin. R4 – Reposition the Tissue

As we get older, our tissues start to sag down. Anything we can do to reposition the tissue can help give you a more lifted appearance. Surgically, we can do a facelift (mini, full, or extended) or neck lift. On the minimally invasive side, we can use threads like Silhouette InstaLift, PDO threads, or MyEllevate to internally lift and reposition the tissues.

R5 – Reduce Facial Volume

Sometimes, instead of having volume lost as we age, we develop excess fullness. It can be fullness under our chin (double chin) or in the jowl area. If we reduce the fullness in the double chin region, it’s also going to help define the transition from the lower face to the neck. This can be done with procedures that either dissolve the fat (Kybella), tighten or melt the fatty tissue (AccuTite), remove the fat (Liposuction), or even freeze the fat (CoolSculpting).

Complementary Facelift Procedures

  • Wrinkle Relaxers such as BOTOX and Dysport can enhance results by relaxing the muscles in areas of concern, addressing the presence of crow’s feet, bunny lines, frown lines, and forehead lines.
  • Dermal Fillers and Sculptra can also help to enhance results by non-surgically replacing volume where it has diminished due to the aging process.
  • Fat Grafting is a surgical technique that involves removing excess fat from a “donor area” where there is plenty to spare (typically the flanks, hips, or abdomen). It is then purified and injected into the treatment area for safe and effective volume restoration with your body’s own tissues.
  • Jawline contouring can be achieved with non-surgical treatment modalities like Kybella, AccuTite, and FaceTite, all of which help to tighten tissues and reduce the presence of excess facial volume.
  • Tissue Repositioning can be non-surgically enhanced with MyEllevate. Additionally, a neck lift or brow lift can be performed alongside your facelift for a truly comprehensive outcome.
  • Facial resurfacing with chemical peels, microneedling, CO2 laser resurfacing, or radiofrequency microneedling can help to bring your results home by creating a glowing and youthful look that complements your newly enhanced facial structure.

Recovery After Your Facelift

After your facelift, you can expect about two weeks of social downtime. We’ll provide you with everything you need to manage your discomfort as you recover, and you’ll be delighted to know that Dr. Cho employs techniques that minimize pain and discomfort during the healing process. In most cases, bruising and swelling have mostly subsided after about two weeks. For the first 24 hours after your facelift, a bulky dressing will cover your incisions, usually located along the hairline, down the border of the ear and behind the ear, and sometimes under the chin. The dressing, along with drains that may have been placed to reduce the fluid collection, will be removed during your first post-operative visit, the day after your surgery. 

Dr. Cho and his team will see you periodically to remove sutures and check on your healing progress during the next one to two weeks. It is important to treat the operative area with care during this initial recovery period, and you will be instructed to avoid having your hair treated with chemicals for three to four weeks following surgery. After four to eight weeks, your healing will be mostly complete, and temporary swelling, bruising, and numbness should be fully resolved. While it may take several months for your final results to be fully realized, the results are well worth the wait. Before your procedure, we’ll give you a detailed recovery timeline that lets you know exactly what you can expect.

You’ll start to notice your final results setting in after about three months, at which point you’ll start to glimpse your long-term results. You’ll notice changes in your appearance with every passing day and week as your muscles and tissues gradually settle into their new position and your final results begin to truly take shape. Depending on the specifics of your treatment plan, we’ll be scheduling follow-up appointments with you at various intervals. This enables us to ensure that you’re remaining comfortable and healing nicely, answer any questions you might have, and generally ensure that you’re getting ready to absolutely love your final results.

What alternative procedures do you offer?

Alternatives to a full facelift include a mini-lift, a lower facelift, or a neck lift. These procedures target the lower part of the face and the neck. A thread lift, the “one-hour facelift” or “lunchtime lift,” is a non-surgical option that boosts collagen levels through the use of threads that are passed under your skin. There are no incisions with the thread lift.

Other non-surgical options include ultrasound therapy, radiofrequency therapy, and injectable fillers such as Restylane or Juvéderm. Platelet-rich plasma therapy is commonly called the Vampire Facelift and uses growth factors derived from your own blood in combination with hyaluronic acid fillers. During your consultation, Dr. Cho will discuss your best options with you and guide you to the procedural choices that are right for you. Most patients are able to return to their normal routines after a recovery period of about three to four weeks.

Your initial consultation with Dr. Cho will cost $100, which will be applied to the cost of your procedure should you decide to move forward with Integrated Aesthetics.

I do not want scars at my ear or temple area from a facelift. Any suggestions?

In general, a surgical facelift provides the most ideal contouring for someone with moderate or severe skin laxity. This procedure can also tighten your underlying musculature and remove excess skin laxity to leave you with a more attractive contour. In some instances, liposuction can also be utilizes to reduce the presence of submental fullness.

There are many post-surgical steps that can be taken to reduce the appearance of scarring, but they will generally not fully eliminate the presence of a scar. These solutions include laser resurfacing, microneedling, silicone sheets, sunscreen, and more. There are also a number of effective, non-surgical options to help address scarring. We recommend Ulthera to patients who are looking for a solution with no downtime, or Profound for those who are comfortable some post-procedural downtime.

Ulthera is an ultrasound-based procedure that heats the underlying tissue and stimulates collagen production. Profound is a radio frequency microneedling device that allows for the creation of elastin, collagen, and hyaluronic acid, restoring some of the lost elasticity in the face. Profound comes with more visible after effects, which can include the possibility of some swelling and bruising. These side-effects will diminish in about a week’s time. The addition of dermal fillers can also enhance your non-surgical results. None of these non-surgical options leave a scar.

How much swelling should I expect after facelift surgery?

Swelling and firmness in the tissues will return to normal as time passes. Between the first and second week, much of your swelling and bruising will dissipate. We generally recommend patients use constant compression for the first week. For the second week, we encourage patients to utilizes compression whenever they can, for about 12 hours per day, when possible. It may take more than a month or two for your tissues to fully soften and any residual swelling to resolve. Lymphatic massage maybe an option to help clear the swelling and firmness at a faster pace. I would make sure that you continue to follow up with your plastic surgeon during this period of time.
